Does Minnesota require an SR-22?

Checked against official Minnesota sources · Last reviewed October 5, 2026

Minnesota doesn't use the SR-22 form. If your license or registration was revoked for driving without insurance or not showing proof of insurance, your insurance company must complete Minnesota's own Insurance Certification (form PS35016) before you can be reinstated.

Driver and Vehicle Services (DVS), part of the Department of Public Safety, handles it. You must keep the insurance for one calendar year. [1][4]

Does Minnesota require an SR-22?

No. Minnesota doesn't use the SR-22. It uses its own insurer certificate, Insurance Certification (PS35016), described below. [1]

What Minnesota requires instead

To reinstate after a no-insurance revocation, your insurance company must complete Minnesota's Insurance Certification (form PS35016). [1]

Form: Minnesota Insurance Certification, DPS form PS35016, completed by your insurance company [1]

How long do you need it in Minnesota?

Insurance must be maintained for one calendar year. By statute the commissioner may require the certificate to be noncancelable for up to one year. [1]

What does reinstatement cost?

$30 reinstatement fee after a revocation for no insurance or failure to produce proof of insurance. DWI revocations cost $680 ($250 fee + $430 surcharge); suspensions cost $20. [2]

What coverage do you need?

$30,000 bodily injury per person, $60,000 bodily injury per accident, $10,000 property damage per accident [3]

Don't own a car?

Yes. If you don't own a vehicle, you can meet the requirement by being named as a driver on an existing policy or by buying a non-owner's or operator's policy. [1]

Who has to file one?

A conviction for driving or owning a vehicle without the required insurance, a revocation for failing to produce proof of insurance, or a DVS finding that required insurance wasn't maintained (for example, from a crash report). Ignition interlock participants with an insurance-related incident on their record also need a certificate. [4]

Who handles it

Minnesota Department of Public Safety, Driver and Vehicle Services (DVS), 445 Minnesota Street, St. Paul [1]

Common questions

Do I need an insurance certificate after a DWI?

Not usually. Minnesota's DWI reinstatement law doesn't list one. If you're in the ignition interlock program, you normally submit a regular insurance card, and DVS asks for a certificate only if your driving record shows an insurance-related incident. [5][6]

Who fills out form PS35016?

An authorized representative of your insurance company completes it, not your insurance agent. DVS also accepts insurance submissions online. [1][7]

Can DVS suspend my license without a conviction?

Yes. If department records, such as a crash report, show you didn't have the required insurance, DVS can suspend your license and revoke your registrations without a hearing. You then need a certificate before reinstatement, and DVS may require one for every vehicle you own. [4]

How much does it cost to get my license back?

The reinstatement fee is $30 after a revocation for no insurance or not showing proof, and $20 after a suspension. A DWI revocation costs $680 each time ($250 plus a $430 surcharge), and a driver's license agent adds a filing fee if you pay through one. After a no-insurance revocation you generally must also pass a driver's license exam. [2][8]

I don't own a car. How do I meet the requirement?

You can be named as a driver on an existing policy, or buy a non-owner's or operator's policy. [1]
